All co-payments, deductibles, co-insurances, and payments for non-covered or cosmetic procedures are due at the time of service. We accept cash, checks, VISA, Mastercard and Discover.

In the event that you need to cancel your appointment, kindly provide 24 hours notice to avoid a $25 cancellation fee for office visits and $50 cancellation fee for surgeries. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment.

All patients under 18 years of age must be accompanied by a parent or legal guardian at their first visit.

In order to better serve you, please bring the following to your appointment: current insurance card, photo I.D., credit card or other preferred form of payment, completed registration/history forms (new patients), all medications (including herbal supplements, vitamins, and creams), pharmacy information and referring physician information.

We utilize an electronic medical records system which replaces the traditional, often unorganized, paper chart. This secure system improves patient care while protecting confidentiality. Among its many advantages, our system facilitates prompt communication with your referring physician, allows prescriptions to be electronically transmitted to your pharmacy to save you valuable time, and guarantees that your medical information will be accurately stored.

For your convenience, patient registration and medical history forms may be downloaded from your home. Completion of these forms prior to your first visit is recommended, as it will decrease your wait time in the office and allow us to serve you sooner. Please fax your forms back to us at (703) 723-9752 before your appointment or arrive 10 minutes early with your completed forms.
