Attention Patients with HMO or POS insurance plans.
We kindly ask that you contact your insurance company prior to your appointment, to verify if a referral is needed to see your dermatologist (specialist). It is your responsibility to obtain a referral from your Primary Care Provider, and to keep it active, if your plan requires it. If you do not have an active referral, we will have to reschedule your appointment.
Attention Patients with Health Insurance.
Please bring your insurance card with you to every appointment. Also, we kindly ask that you have an understanding of your health insurance plan prior to receiving an evaluation or treatment with your doctors. Payments for deductibles/coinsurance/copays are your responsibility and will be collected at the time of your visit. If you have questions regarding your insurance coverage as it pertains to your visit with us, please feel free to ask us upon arrival.
All co-payments, deductibles, co-insurances, and payments for non-covered or cosmetic procedures are due at the time of service. We accept cash, checks, VISA, MasterCard and Discover. In the event of outstanding balances, you can make an online payment by clicking the “Make a Payment” button below to be directed to our secure payment page. Please enter the patient account number printed on your statement in order to complete the online payment. You will receive an email confirmation and will be redirected back to our website following a completed transaction.
Cosmetic/Laser Return Policy
No refunds are made for products, treatment packages or pre-paid treatments once they are purchased. If for some reason you are not able to use an un-rendered, pre-paid service, you may do a one time exchange of the unused portion toward other services. Unopened products may be exchanged for alternative products within 7 days of purchase.
Cosmetic/Laser Treatment Expiration
All treatment packages and pre-paid treatments must be used within 12 months of date of purchase or they will expire without refund or exchange.
